Information we collect
When you request a consultation, we collect the information you provide, including contact information, the patient’s name and birthdate, the reason for the consultation, and the preferred appointment time. We also use limited technical information to protect the form, prevent abuse, and understand website performance.
How we use information
We use consultation-request information to review the request, coordinate scheduling, respond to questions, operate the website, maintain security, and meet applicable professional and legal obligations. Submitting a request does not create a doctor-patient relationship or confirm an appointment.
Text-message consent
Providing a mobile number is required so the office can process an appointment request, but agreeing to receive text messages is optional. We send transactional appointment-request and scheduling texts only when the separate text-message consent box is selected. Message frequency varies, message and data rates may apply, and recipients may reply STOP to cancel or HELP for help.
Mobile opt-in information is not shared with third parties or affiliates for marketing or promotional purposes.
We may use service providers only to operate requested communications and related systems, subject to appropriate safeguards. Text-message consent is never inferred from general permission to call or email.
Retention
A complete consultation request is scheduled for automatic deletion 12 months after submission unless a documented legal hold applies. If the person is seen, the official patient chart remains governed by the practice’s existing record process. If the person is not seen, no patient chart is created from the website request.
Minimal, non-clinical evidence of an affirmative text-message consent is stored separately for seven years from collection. Minimal opt-out suppression information is retained while the messaging program operates so that a prior opt-out is honored.
Sharing and security
We do not sell consultation-request information. We disclose information only as needed to operate the practice and its systems, comply with law, protect rights and safety, or complete a practice-authorized transition. We use administrative, technical, and contractual safeguards appropriate to the information involved.
