Information we collect

When you request a consultation, we collect the information you provide, including contact information, the patient’s name and birthdate, the reason for the consultation, and the preferred appointment time. If you choose to provide them, we also collect a mailing address and dental-insurance details. Those optional sections may be left blank or partially completed. We do not request a Social Security number through this form. We also use limited technical information to protect the form, prevent abuse, and understand website performance.

Website analytics may include pages viewed, referring and outbound links, visit timing, interaction and scroll measurements, campaign tags, device category, browser, operating system, language, timezone, screen dimensions, and coarse city, region, or country information when supplied by the website hosting service. We do not place IP addresses or consultation-form answers in the analytics view.

The site creates a random first-party session identifier so pages and actions during the current visit can be grouped together. It does not contain a person’s name or contact information, is stored only for the browser session, and is not used to recognize future visits. When a consultation request succeeds, the site records the request’s opaque identifier with the completion event so authorized practice staff can view that website journey beside the request. Contact details, patient information, birthdates, consultation reasons, and other form answers are not copied into the analytics record.

How we use information

We use consultation-request information to review the request, coordinate scheduling, respond to questions, operate the website, maintain security, and meet applicable professional and legal obligations. Submitting a request does not create a doctor-patient relationship or confirm an appointment.

Website and advertising measurement

We use first-party website analytics to understand traffic sources, page use, and appointment intent without placing advertising cookies. Google Ads uses consent mode with advertising storage denied by default. If you allow ad measurement, Google Ads and Meta may place or read cookies and receive limited device, page, and campaign information so we can measure advertising performance.

After a request is successfully saved, Google Ads and Meta may receive a conversion signal without the request identifier or any form answers. We do not send names, email addresses, phone numbers, birthdates, appointment times, treatment information, or consultation reasons to either service. You can allow, decline, or revisit ad measurement at any time using the website’s Privacy choices control.

Text-message consent

Providing a mobile number is required so the office can process an appointment request, but agreeing to receive text messages is optional. We send transactional appointment-request and scheduling texts only when the separate text-message consent box is selected. Message frequency varies, message and data rates may apply, and recipients may reply STOP to cancel or HELP for help.

Mobile opt-in information is not shared with third parties or affiliates for marketing or promotional purposes.

We may use service providers only to operate requested communications and related systems, subject to appropriate safeguards. Text-message consent is never inferred from general permission to call or email.

Retention

A complete consultation request is scheduled for automatic deletion 12 months after submission unless a documented legal hold applies. If the person is seen, the official patient chart remains governed by the practice’s existing record process. If the person is not seen, no patient chart is created from the website request.

Minimal, non-clinical evidence of an affirmative text-message consent is stored separately for seven years from collection. Minimal opt-out suppression information is retained while the messaging program operates so that a prior opt-out is honored.

Sharing and security

We do not sell consultation-request information. We disclose information only as needed to operate the practice and its systems, comply with law, protect rights and safety, or complete a practice-authorized transition. We use administrative, technical, and contractual safeguards appropriate to the information involved.
